I, as a volunteer for the special needs fishing derby hereby release, indemnify, and hold harmless Hook-A-Heart Foundation, sponsors, owners and lessors of premises and property used to conduct the event ("releasees"), with respect to any and all injury, disability, death, or loss or damage to person or property, which I suffer or which I am found to have caused in whole or in part by my negligence or intentional misconduct, whether arising from the negligence of the releasees, or otherwise, to the fullest extent permitted by law.
For permission to participate in the special needs fishing derby, I hereby grant Hook-A-Heart Foundation on behalf of myself and accompanying child the unconditional right to use my voice, photographic likeness, including but not limited to video/audio productions, articles, and/or press releases, without restriction as to changes or alterations at any time. I understand that I will not be entitled to receive any royalties or other compensation in connection with such use and waive my right to review the finished product.
As a volunteer for the special needs fishing derby I agree to treat all volunteers, participants, and community members with respect, courtesy and dignity. I will avoid insensitive or offensive language. I will follow all rules and policies set forth by a event coordinator or supervising volunteer. Using offensive language or behavior may be grounds for removal from event. I agree to not to be alone with a child during the event unless I am the parent or guardian of that child. I will refrain from using all tobacco products at the event. I will refrain from drinking alcohol during the event.
